Injections

One feature of our program is our pride in providing intra-articular and trigger point muscle injections at the bedside including but not limited to Subacromial bursa steroid injections for Rotator Cuff tendinopathy, Trapezius muscle Trigger Point Injections, lumbar paraspinal muscle trigger point injections, Trochanteric bursitis steroid injections, and Intrarticular Steroid Knee injections for Osteoarthritis and we find with these injections that we are many times able to eliminate the need for oral pain medications that come along with all their systemic side effects. And while we perform these procedures in-house, we always make sure to receive informed consent, and perform the procedure in a sterile manner. Thankfully, many of our patients have found much satisfaction with this program.

 

Pain Management

We currently work in SNFs in both Long Term and Short Term units providing pain management through many means. We always practice the mantra of "Do no harm" and "Start Low, Go Slow", so we like to avoid heavy medications whenever possible, but when necessary we specialize in prescribing the entire spectrum - everything from Tylenol, to Voltaren gel, to Mobic, to Tramadol, to Dilaudid. If patients are on blood thinners we always stay away from NSAIDs to avoid bleeding risks, and if a patient has any history of substance abuse, we like to practice the recommendation of safe, non-pharmacologic modalities for pain control if possible.
 

Strokes/TBI

For cases of spasticity secondary to Stroke, TBI etc, we are proficient in prescribing antispasmodic medications inclusive of Baclofen and Zanaflex etc.  

Post Acute Pain

Many short term post acute patients may experience orthopedic pain. Such scenarios often call for common protocols of low dose oxycodone prescriptions as is common after such procedures such as joint replacements and fixation procedures. Also commonly and concomitantly, patients will complain of sciatica and neuropathic pain for which we commonly find good outcomes with the use of Gabapentin and Lyrica tailored to the type of neuropathy that patient presents with. Along with the prescription of opioid medications, we also practice thorough education of nurses and include in our documentation a full list of precautions when prescribing any medications that come with side effects to be monitored for.
 

Teamwork

Our team know how to work well together with the primary physician, nurses and therapists in a collaborative way and in doing so this also allows for a smoother rehab course and again, better outcomes for all.  We never look to step on the toes of any of the attending physicians. We only look to aid and facilitate between the medical, therapy, and nursing departments and only ever offer recommendations to make life easier. Again our services are completely free of charge for the facility. Thankfully we have found this business to thrive because such a model has only ever proven to enhance a facility's marketing and reviews and being that Long Island is our home, it really would be our pleasure to serve you. 

 

Documentation

Our documentation always includes a full set of precautions and with all our consultations and recommendations we always instruct to please have all recommendations approved with the primary attending physicians prior to ordering.
